About The Position

The Chrysalis House is a group home where young pregnant or parenting teens age 16-21 receive basic support including housing, counseling, and parenting training for up to 18 months. The Relief Counselor will monitor and provide support for residents in the event the On-Site Support Staff and/or Skill builders are unavailable for personal emergencies or scheduled vacation. The Chrysalis House Case Manager will be assigned pregnant or parenting clients from both the residential and non-residential tracks of the Transitional Living Program. This position will provide intensive case management, including basic and independent living skills assessment, training, referrals, and other supports as needed, as well as assist in implementing the parenting training and engagement across the TLP Program.

Requirements

  • Bachelor's Degree in Human Service or related field AND 3 years' related experience OR equivalent combination
  • A valid drivers license
  • Strong literacy and communication skills
  • Knowledge of community resources
  • Advanced Knowledge of Microsoft Office
  • Advanced knowledge of Internet Explorer

Nice To Haves

  • Master's Degree preferred

  • Develop, coordinate and facilitate parenting skills curricula, incorporating CYS philosophy and effective teaching approaches
  • Crisis intervention and response
  • Outreach, screening, intake and assessment
  • Coordination, development, implementation, ongoing assessment of client's case management needs, service and plans
  • Referral readiness, information and facilitation
  • Case specific youth advocacy
  • Transporting program participants
