The Center at Grande is a skilled nursing facility that serves patients needing physical rehabilitation and/or complex nursing care. Requirements
- Excellent verbal and written communication with professional phone etiquette.
- Customer service oriented
- High attention to detail
- High School diploma or GED
- Must be able to pass a pre-employment background check and drug screen
Nice To Haves
- Minimum of 1 year Case Management experience preferred
Responsibilities
- Track and record patient case information, progress, and evaluate recovery.
- Meet with patient and their families to understand their needs and recommend appropriate care with treatment plans, while providing support and reassurance.
- Work with team members to ensure discharge planning goals and objectives are developed.
- Schedule appointments, and coordinate care
