Our Enhanced Care Management (ECM) program delivers a whole-person, interdisciplinary approach to care, addressing the clinical and non-clinical needs of clients with complex medical and social conditions. The program focuses on: Outreach and Engagement Comprehensive Assessment & Care Planning Enhanced Coordination of Care Health Promotion Transitional Care Services Member and Family Support Community Resource and Social Service Referrals The ultimate goal is to improve care coordination, address social determinants of health (SDOH), and reduce unnecessary healthcare utilization for high-need Medi-Cal beneficiaries. Reporting to the Program Manager, the Case Manager delivers high-quality, client-centered case management services for individuals and families experiencing homelessness. You will manage a caseload of 15–20 participants, serving as their central point of contact. We are deeply invested in staff development. This role provides a clear advancement pathway—from Case Manager to Lead Case Manager and ultimately to Program Manager. We provide mentorship, resources, and tools for your continued professional growth. Requirements

  • Strong commitment to The Good Seed CDC’s mission and values
  • Knowledge of homelessness, poverty, and systemic barriers
  • Experience working with vulnerable populations, especially those with disabilities
  • Familiarity with Los Angeles County service systems
  • Flexibility to work some evenings and weekends
  • Valid California Driver’s License, auto insurance, and reliable transportation

Responsibilities

  • Conduct initial and quarterly psychosocial assessments
  • Collaborate with participants on individualized service plans and housing stability goals
  • Build trusted, professional relationships with clients
  • Provide outreach, housing navigation, and ongoing support
  • Facilitate referrals to healthcare, mental health, substance use, and community services
  • Support clients in obtaining employment, benefits, and education opportunities
  • Accompany clients to key appointments as needed
  • Use a harm reduction, strength-based, and client-focused approach
