Case Manager: Full Time
About The Position
Cascade Medical is seeking a full-time Case Manager to coordinate patient care and discharge planning across inpatient, observation, and Swing Bed services. The role works with patients, families, providers, nursing, rehabilitation, social work, registration, and community partners to identify barriers early, support appropriate admissions and transitions, and promote safe, timely, and well-coordinated care. The Case Manager maintains clear documentation and helps patients receive the services, equipment, follow-up, and information needed after discharge. The anticipated schedule is 40 hours per week, Monday through Friday, with occasional weekend shifts. Exact scheduled hours will be determined with the director upon hire. This position works onsite. As a safety-sensitive position, employment requires a pre-employment drug screen, including testing for THC.
