Case Manager: Full Time
About The Position
Cascade Medical is seeking a full-time Case Manager to coordinate patient care and discharge planning across inpatient, observation, and Swing Bed services. The role works with patients, families, providers, nursing, rehabilitation, social work, registration, and community partners to identify barriers early, support appropriate admissions and transitions, and promote safe, timely, well-coordinated care. The Case Manager maintains clear documentation and helps patients obtain the services, equipment, follow-up, and information they need after discharge. The position is onsite and anticipated to work 40 hours per week, Monday through Friday, with occasional weekend shifts. Exact scheduled hours will be determined with the director upon hire. As a safety-sensitive position, it requires a pre-employment drug screen, including testing for THC. Applicants should submit an online application, resume, and cover letter of interest.
