About The Position

The Case Manager coordinates interdisciplinary care for a designated patient population. The role reviews inpatient admission records to support appropriate use of hospital resources and determine whether patients are admitted at the appropriate level of care. The Case Manager assesses discharge needs, coordinates suitable discharge plans, and collaborates with the interdisciplinary team to achieve desired financial and quality outcomes. The position also maintains and enhances payor relationships. Shifts are primarily Monday through Friday, with weekends as needed. The role is PRN at the Denver LTACH.

Requirements

Current BLS certification is required.
Three to five years of inpatient experience; experience in an acute, IRF, or LTACH setting is preferred.
Licensure in the state where the hospital is located is preferred.
The designated case manager may be an occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
