About The Position

The Case Manager coordinates care across an interdisciplinary team for a designated patient population. The role reviews inpatient admission records to ensure appropriate use of hospital resources and determine whether admission is at the appropriate level of care. The Case Manager assesses discharge needs, coordinates suitable discharge plans, and collaborates with the interdisciplinary team to achieve desired financial and quality outcomes. The role also maintains and enhances relationships with payors.

Requirements

• Current BLS certification.
• Three to five years of inpatient experience.

Nice To Haves

• Licensure in the state where the hospital resides is preferred.
• Experience in an acute, IRF, or LTACH setting is preferred.
• Professional discipline may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
