About The Position
The Case Manager is responsible for coordinating interdisciplinary care for a designated patient population. The role reviews inpatient admission records to support appropriate utilization of hospital resources and determine whether patients are admitted at the appropriate level of care. The Case Manager assesses discharge needs and coordinates appropriate discharge plans, working collaboratively with the interdisciplinary team to achieve desired financial and quality outcomes. The role also maintains and enhances relationships with payors.
Requirements
- Current BLS certification is required.
- Three to five years of inpatient experience.
- Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
Nice To Haves
- State licensure where the hospital resides is preferred.
- Inpatient experience in an acute, IRF, or LTACH setting is preferred.
