Case Manager PRN | Denver LTACH
About The Position
The Case Manager coordinates interdisciplinary care for a designated patient population. The role reviews inpatient admission records to support appropriate use of hospital resources and determine whether admissions are at the appropriate level of care. The Case Manager assesses discharge needs, coordinates suitable discharge plans, and collaborates with the interdisciplinary team to support desired financial and quality outcomes. The position also maintains and enhances payor relationships. Shifts are primarily Monday through Friday, with weekends as needed. The listed pay is up to $48.00 per hour. The position is PRN at PAM Health’s Denver LTACH.
Requirements
- Current BLS certification is required.
- Three to five years of inpatient experience; experience in an acute, IRF, or LTACH setting is preferred.
- Professional discipline may include occupational therapy, physical therapy, registered nursing, respiratory therapy, speech therapy, or licensed social work.
