About The Position

The Steven A. Cohen Military Family Clinic at The Up Center provides high-quality, targeted mental health services to military veterans who served in the Iraq and Afghanistan wars, active-duty service members, and their family members. Our mission is to help individuals and families move from crisis toward stabilization, wellness, and long-term resilience. The Case Manager serves as both a clinical support professional and a community outreach liaison, helping bridge military-connected individuals and families to essential behavioral health, wellness, and community resources throughout Hampton Roads and across Virginia. This role places a strong emphasis on outreach, partnership cultivation, community engagement, advocacy, and relationship management to ensure clients can access comprehensive and coordinated support systems. The Case Manager actively develops and maintains collaborative relationships with community organizations, veteran-serving agencies, healthcare systems, schools, social service providers, and military-connected networks to strengthen referral pathways and improve access to care. This position also represents the clinic within the community through outreach events, presentations, coalition meetings, and partnership-building initiatives designed to increase awareness of clinic services and reduce barriers to mental health support. A strong candidate for this role will demonstrate exceptional interpersonal communication skills, cultural responsiveness, trauma-informed engagement practices, and a passion for serving military-connected individuals and families. The ideal candidate is relationship-driven, highly collaborative, adaptable, and committed to strengthening community connections that improve client outcomes.

Requirements

  • Master’s Degree in counseling, marriage and family therapy, social work, clinical psychology, or related field
  • 1+ years case management experience in a mental health setting, preferably working with veteran and military populations
  • 2+ years in a customer service-focused environment.
  • Knowledge of/experience working with local social service, public health, and VSO organizations and resources.
  • Demonstrated leadership capacity and ability to foster team building and a supportive, flexible environment.
  • Flexibility to work independently or as part of a team, anticipated problems, generate solutions, and demonstrate flexibility and sensitivity when engaging with staff and clients.
  • Work flexible hours to include 1-2 evenings per week.
  • Demonstrated knowledge of HIPPA.
  • Ability to juggle multiple opportunities to drive results on several important projects simultaneously; excellent organizational and time-management skills.
  • Excellent written and oral communication skills.
  • Excellent computer skills and command of Word and Excel, HER experience preferred.
  • Must have a valid driver’s license and meet background check.

Responsibilities

  • Develop, strengthen, and maintain collaborative partnerships with community organizations, veteran-serving agencies, healthcare providers, schools, military-affiliated organizations, housing agencies, peer-support programs, and social service networks.
  • Serve as a community ambassador for the clinic by attending outreach events, coalition meetings, resource fairs, military and veteran events, and community engagement opportunities.
  • Conduct presentations and educational outreach to community partners, stakeholders, and service providers regarding clinic services, referral processes, and available resources.
  • Identify gaps in community resources and proactively cultivate new partnerships and referral opportunities to better support clients’ evolving needs.
  • Foster ongoing communication and engagement with referral partners to ensure continuity of care, collaborative problem-solving, and high-quality client support.
  • Support community awareness initiatives that reduce stigma surrounding mental health and increase access to trauma-informed care and supportive services.
  • Assist in expanding outreach efforts to underserved and rural military-connected populations.
  • Build a visible and active presence in the community, effectively engaging diverse stakeholders across healthcare, behavioral health, education, military, and social service systems.
  • Provide comprehensive case management services via telehealth and in-person to veterans, service members, and their families enrolled in clinic services.
  • Conduct needs assessments and facilitate warm handoffs to community resources and supports.
  • Assist clients in navigating complex systems of care, including healthcare, education, housing, employment, legal services, and benefits coordination.
  • Collaborate with multidisciplinary clinical staff to ensure coordinated, person-centered care planning and service delivery.
  • Maintain accurate and timely documentation within the electronic health record system in compliance with HIPAA and agency policies.
  • Conduct follow-up communication and engagement with clients awaiting services or transitioning from care.
  • Participate in multidisciplinary team meetings, supervision, case consultation, and care coordination activities.
  • Support crisis intervention and risk management efforts in collaboration with clinical staff and leadership.
  • Provide limited direct clinical services, including individual, family, or couples therapy as assigned, utilizing evidence-based and trauma-informed approaches.
  • Perform and document client screenings, psychosocial assessments, and progress in treatment.
  • Engage with clinic’s data manager to foster a positive relationship around data collection, trend analysis, research and reporting.
  • Serve as a backup to the front desk and intake (as needed).
  • Administrative duties as assigned.
