This role involves coordinating and managing program services for assigned OPARC participants by developing and implementing Person Centered Plans (PCP) and Individual Habilitation Service Plans (IHSP), conducting regular meetings and visits, monitoring progress, maintaining records, and providing counseling and advocacy.
