Under the direct supervision of the Social Services Program Manager, the Social Services Case Manager provides housing-focused, participant-centered case management and supportive services to individuals and families enrolled in CRC programs or those seeking services and resources. The Case Manager supports participants in identifying and addressing barriers to housing stability, increasing self-sufficiency, and working toward individualized goals. Services include case management, crisis intervention, emergency assistance, housing navigation and stabilization, and facilitation of homelessness prevention and intervention programs, including Rapid Rehousing. Case Managers also assist participants with employment readiness, life skills training, financial education and budgeting, benefits enrollment, resource navigation, and individualized goal setting. This is a community-based position that requires flexibility as to where services are provided. Case Managers are expected to meet participants where they are, which may include CRC offices, participants’ homes, community locations, housing units, partner agencies, and other locations. Regular local travel and reliable transportation are required.
