About The Position

The Case Manager role is within the Supported Housing Program and is responsible for facilitating the lasting rehabilitation of residents. This involves initial and ongoing assessments, monitoring functioning, developing and reviewing individualized service plans, and providing supportive services. The Case Manager also acts as an advocate for residents, conducts routine home visits, and provides training in areas such as daily living skills and medication monitoring. Case managers maintain an active caseload and report to the Program Director.

Requirements

  • Bachelor Degree in a human service field.
  • Related Experience.
  • Experience working with people with Mental Illness.

Responsibilities

  • Participate in the intake of people for admission to the supported housing program and in the initial assessment.
  • Conduct at least monthly assessment of residents to determine functioning, needs, and strengths.
  • Develop initial individualized service plan within 28 days after admission with residents, involving resident family and other collaterals as necessary.
  • Ensure objectives and services in the service plan relate to goals and are relevant for the resident. Provide skill training as necessary.
  • Record resident progress within 25 hours of services delivery and in accordance with the individualized service plan, or within 25 hours of contact not related to the plan. Record telephone conversations immediately.
  • Ensure resident case records are current and maintained in accordance with organization and accreditation requirements.
  • Make referrals to local supportive service providers, act as a liaison, escort residents to appointments, and meet with providers to ensure continuity of care.
  • Attend and participate in case conferences at the program site and with other supportive service providers.
  • Assess income assistance needs, advise residents of program requirements, assist with applications, accompany residents to offices, and advocate for timely receipt of benefits. Stay updated on income assistance program changes.
  • Respond to resident crises, emergencies, and situations requiring immediate staff involvement.
  • Maintain a positive working relationship with residents and encourage their full participation in the program.
  • Maintain a positive working relationship with local supportive service providers and ensure resident confidentiality.
  • Attend conferences, meetings, and educational seminars as required by the Program Director.
  • Act as an advocate for residents and plan/implement group social/leisure/cultural activities.
  • May participate in the in-service training of all staff.
  • Perform related duties as required by the immediate supervisor.
  • Review service plans with residents at least semi-annually and revise as necessary, involving resident family/significant other and collaterals with resident approval.
