About The Position

Greater Nashua Mental Health is seeking a full-time Case Manager to join its Adult Services Team. The Case Manager provides direct support, education, referrals, and outreach to a caseload of adults diagnosed with severe mental illness. The role involves meeting with individuals in their natural environments and providing case management services that help clients lead full and satisfying lives. The team offers mentoring opportunities to support professional growth.

Requirements

Bachelor’s degree in social work, psychology, rehabilitation, education, or a related human services field; alternatively, an associate’s degree in one of these fields plus either two years of experience working with people with a serious emotional disturbance or serious mental illness, or two years of experience that provides an understanding of mental illness. Applicants in the process of obtaining a degree may also be considered.
Familiarity with community resources.
Knowledge of and ability to use Microsoft Office products, including Word, Outlook, and Excel.
Strong collaboration skills and ability to work effectively both independently and as part of a team.
Comfort working in a fast-paced environment with frequent deadlines; high energy, self-motivation, and a proactive approach.
Active driver’s license and an automobile with liability insurance meeting the agency’s minimum amounts.

Responsibilities

Collaborate with clients to develop care and support plans that maximize their control and choice in meeting care needs.
Monitor factors affecting mental health, including housing, transportation, and vocational needs.
Connect clients with and refer them to resources outside the mental health center, including medical, educational, and social services.
Work as part of an integrated team and communicate with colleagues, collateral contacts, and supervisors about client care.
Provide direct and indirect case management and functional support services.
Complete treatment plans, Adult Needs and Strength Assessments (ANSA), Case Management Care Plans (CMCP), and Quarterly Reviews.
