About The Position

The Case Manager is a member of Ava Health’s interdisciplinary treatment team and helps clients address practical, social, and environmental barriers that may impact treatment and long-term recovery. The Case Manager works closely with clients and their treatment team to coordinate community resources, address social determinants of health, support discharge and aftercare planning, and help clients develop the skills necessary to navigate healthcare and community systems independently.

Requirements

  • Bachelor's degree in social work, psychology, human services, behavioral health, or a related field preferred but not required; relevant case management or behavioral health experience may be considered in lieu of degree requirements.
  • Experience working in behavioral health, human services, case management, or a related setting preferred.
  • Strong communication, organization, and interpersonal skills.
  • Ability to manage multiple client needs, referrals, appointments, and deadlines.
  • Ability to work effectively as part of an interdisciplinary team.
  • Comfortable working with individuals experiencing mental health and/or substance use disorders.

Nice To Haves

  • Behavioral health or substance use treatment experience.
  • Experience with PHP, IOP, outpatient, or residential services.
  • Knowledge of community resources, Medicaid, benefits, housing, transportation, and other support services.
  • Experience with discharge and aftercare planning.
  • Familiarity with Motivational Interviewing and person-centered care.

Responsibilities

  • Maintain an assigned case management caseload.
  • Complete case management and social determinants of health assessments.
  • Meet regularly with clients to identify needs, establish goals, and address barriers to treatment and recovery.
  • Assist clients with housing, transportation, insurance, benefits, employment, education, healthcare, legal obligations, and other community resources.
  • Coordinate referrals and appointments with outside providers and community organizations.
  • Collaborate with Primary Clinicians, psychiatric providers, nursing, residential staff, and other members of the treatment team.
  • Participate in treatment team meetings and communicate relevant client needs, barriers, and progress.
  • Coordinate discharge and aftercare planning, including follow-up appointments, community resources, and continuity of care.
  • Encourage client independence, self-advocacy, accountability, and effective problem-solving.
  • Complete case management notes, assessments, aftercare plans, and other required documentation accurately and on time.
  • Maintain appropriate professional boundaries, confidentiality, and releases of information.
  • Recognize clinical or safety concerns and promptly communicate them to the appropriate clinical or medical team member.
