Case Manager
About The Position
Tribute Home Care is looking for a customer-service focused, dedicated and collaborative Case Manager to join our Chicago team. This is a great position for someone with strong case management experience, leadership experience, and a passion for delivering exceptional service and fostering strong referral relationships. Tribute Home Care is a growing private home care company that offers people who need assistance in the home what they want above all else: consistently great caregivers and great service. We do this by hiring only those caregivers who can build real connections with their clients and by offering them the community of support and recognition they deserve.
Requirements
- Bachelor's degree from an accredited college
- At least two years of clinical case management experience, including experience leading or mentoring others
- An eagerness to impact growth
- Unwavering, passionate commitment to the customer experience and service excellence
- Strong writing and communication skills
- Comfort using AI-powered systems, such as ChatGPT, and adopting new AI tools as they emerge
- Ability to travel throughout the greater Chicago area
- Ability to work from Tribute's Skokie office on Wednesdays and attend other in-person meetings as needed
Responsibilities
- Providing clients with a warm and helpful introduction to Tribute through responding promptly to inquiries for new service and setting up and completing in-person assessments
- Overseeing the setup of Tribute's services through formulating client care plans, helping successfully match Caregiver Associates with clients and orienting caregivers to client's care plans
- Providing regular, proactive communication to all parties involved in each client's care
- Revising care plans as needed and monitoring the quality of services provided
- Developing strong and trusting relationships with Tribute's clients and their families
- Building relationships with community referral sources
