CASE MANAGER (BSW)

About The Position
Bloomington Meadows Hospital is dedicated to helping patients restore their health through personalized outpatient and inpatient treatment programs. Its treatment approach emphasizes communication among patients, families, referral sources, and professionals. The Case Manager assists with social services functions, coordinating communication and follow-up care for patients and families.
Requirements
- Bachelor’s degree from an accredited college or university in Social Work or a clinical-related mental health field.
- Strong communication skills.
- CPR certification and HANDLE WITH CARE training within 90 days of employment and before assisting in a restraining procedure.
Nice To Haves
- Two years of experience in a mental health care facility is preferred.
Responsibilities
- Communicate with referral sources, families, physicians, and schools.
- Ensure patients and families are scheduled for aftercare.
- Provide short-term and long-term family therapy.
- Provide information and referrals as needed.
- Conduct wellness checks.
