About The Position
The BSW Case Manager supports residents, families, and care teams with dignity and empathy, helping protect resident rights and improve quality of life. Responsibilities include discharge planning, coordinating calls with doctors’ offices, and completing PA referrals. The position is located at Brentwood Springs in Newburgh, Indiana, and is scheduled Monday through Friday for 28 hours per week.
Requirements
- Bachelor of Social Work (BSW) from a program accredited by the Council on Social Work Education.
Nice To Haves
- Experience in health care, crisis intervention, counseling, and/or a social agency; internships or other employment may count.
- Continuing education toward a Master of Social Work (MSW) is recommended.
