About The Position

Join an exceptional healthcare team and play a vital role in coordinating care, advocating for patients, and ensuring the right care is delivered at the right time. The Case Manager role includes: Coordination and management of patient care throughout the entire continuum, from admission through discharge and post-hospital follow-up. Collaborate closely with patients, families, physicians, interdisciplinary care teams, and third-party payers to ensure appropriate levels of care, seamless transitions, exceptional patient outcomes, and effective utilization of healthcare resources. Serve as a patient advocate while facilitating high-quality, efficient, and compassionate care delivery.

Requirements

License to practice as a Registered Nurse in the State of New York.
Bachelor of Science Degree in Nursing and the equivalent of two (2) years of full time clinical experience in the field of Utilization Management/Discharge Planning, Home Health Care or Case Management; or
Associate’s degree in Nursing and the equivalent of four (4) years of full-time clinical experience in the field of Utilization Management/Discharge Planning, Home Health Care or Case Management; or
Nursing diploma and the equivalent of five (5) years of full-time clinical experience in the field of Utilization Management/Discharge Planning, Home Health Care or Case Management.
