The Doe Fund is seeking a Case Manager for its newest permanent supportive housing program, the Westchester Residence. This program is designed for homeless individuals and families living with HIV/AIDS, providing long-term residential care and twenty-four-hour support services in a community environment. The goal is to maximize residents' independence while ensuring their physical and emotional security and well-being. The residence will have 151 units, with 31 designated for individuals referred by the New York City HIV/AIDS Services Administration (HASA), and the remaining 120 units for moderate-income households. The Case Manager will be responsible for providing supportive case management services to the HASA-referred residents. The ideal candidate will possess strong clinical and communication skills and experience working with the HIV/AIDS population.
