Case Manager - White Plains Road Residence
About The Position
The Doe Fund is seeking a Case Manager for its newest permanent supportive housing program for homeless individuals living with HIV/AIDS. This 98-unit residence aims to provide long-term residential care and 24-hour support services in a community environment that promotes resident independence, security, and well-being. The program will house 49 individuals referred by the New York City HIV/AIDS Services Administration (HASA) and 49 units for moderate-income households. The Case Manager will be responsible for providing supportive case management services to residents, requiring strong clinical and communication skills, experience with the HIV/AIDS population, and availability for some evening hours.
Requirements
- Bachelor’s Degree in Social Work or relevant field
- Minimum of three years of post-bachelor experience
- Working knowledge of HIV/AIDS, substance abuse, mental illness, and homelessness
- Excellent case management skills, including knowledge of benefits in the NYC Metro area
- Sensitivity to community issues and needs
- Outstanding interpersonal and communication skills, both oral and written
- Strong ethical standards, confidentiality, and personal integrity
- Flexibility with schedule to cover evening activities
- Computer proficiency (i.e., email, Microsoft Word, Excel, and PowerPoint)
Responsibilities
- Maintain a caseload and provide supportive case management services to these individuals
- Maintain resident records in compliance with agency and funding standards
- Conduct and review assessments
- Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency
- Monitor and ensure residents are linked to healthcare providers
- Provide counseling and crisis intervention as necessary
- Assist residents in securing and maintaining benefits
- Maintain a liaison with community services, including hospitals, substance abuse treatment programs, HRA, and mental health programs
- Facilitate groups and activities that promote health and independent living skills development
- Monitor and assist residents with rent payments and budgeting
- Conduct regular unit inspections
- Educate residents about approaches to mental health
- Attend ongoing training to enhance the services provided to the residents
- Assist with the completion of reports as needed
- Other duties as assigned
