About The Position
Safro Staffing is partnering with a behavioral health and addiction treatment organization seeking an experienced Case Manager to assist clients in accessing resources, coordinating services, and supporting successful treatment outcomes. This position serves as a liaison between clients, treatment providers, community agencies, and support systems.
Requirements
- High School Diploma required
Nice To Haves
- Associate's or Bachelor's Degree preferred.
- Experience in behavioral health, substance use treatment, social services, or case management preferred.
- Strong organizational and communication skills.
- Ability to manage multiple priorities.
- Knowledge of community resources and recovery supports preferred.
Responsibilities
- Coordinate client services and discharge planning.
- Assist with aftercare placement and recovery resources.
- Support clients with transportation, housing, employment, and community resources.
- Assist with disability, FMLA, and other documentation needs.
- Coordinate appointments and outside provider referrals.
- Maintain accurate case management documentation.
- Collaborate with clinical and medical staff.
- Advocate for client needs and treatment goals.
- Monitor client progress and barriers to success.
