About The Position

Unity House of Troy is looking for a compassionate individual to join our Community Residence 14th street case management team. Unity House is a Rensselaer County-based human service agency that provides a wide range of services to meet the otherwise unmet needs of people in our community who are hurting and struggling. We assist those who are living in poverty, adults living with mental illness or HIV/AIDS, victims of domestic violence, and children with developmental delays. We work to achieve social justice in our community and to create a better understanding of those we serve.

Requirements

High School Diploma or GED required.
Minimum 1 year of experience working with adults living with mental illness.
Valid NYS Driver License and reliable transportation.
Strong verbal and written communication skills.
Basic computer proficiency.
Ability to work independently and collaboratively as part of a team.

Responsibilities

Foster a structured, supportive, and inclusive environment for residents.
Build positive, professional relationships with consumers.
Assist with daily living skills, including cooking, budgeting, and personal care.
Support residents with medication management.
Ensure the facility remains clean, safe, and well-functioning.
Provide supportive counseling and offer encouragement and positive feedback.
Transport clients and complete program-related errands as needed.
Promote a culture of belonging and respect.
