About The Position

Case management is a method of providing services whereby the case manager assesses the needs of the client and the client's family, when appropriate, and arranges, coordinates, monitors, evaluates, and advocates for a package of multiple services to meet the specific client's complex needs. Case Managers must be highly organized, empathetic, compassionate, able to work with diverse populations, and eager to help vulnerable adults. All applicants must complete a Florida Level 2 background check.

Requirements

  • Must be fluent in both oral and written Spanish and English
  • Must Pass Level 2 background/security screen.
  • One year of social service experience.
  • Must know computer and word processing applications.
  • Must possess a valid Florida driver’s license and reliable vehicle.
  • Must have excellent interpersonal, communication, and organizational skills and the ability to work with a wide variety of groups and individuals.
  • Sufficient skills to be able to read and write effectively in English.
  • Assumes responsibility for professional growth and development.

Responsibilities

  • Conduct client assessment and determine eligibility under program guidelines.
  • Conducts home visits for initial, annual and health assessment.
  • Develop a case plan in accordance with client needs and implement plan by providing, arranging, and coordinating services.
  • Maintain contact with client according to client’s needs and review plan periodically to ensure appropriateness thereof.
  • Provide management of service delivery, contact referral services, and coordinate with other agencies to provide the client with services not met by OCOA programs.
  • Compile and maintain progress notes related to service provision and monitoring.
  • Follow all policies and procedures of Osceola Council on Aging, Inc.
