About The Position

The Denver Regional Council of Governments (DRCOG) is seeking a Case Manager – Bilingual (English/Spanish) to join their team. DRCOG serves as a visionary leader along Colorado’s front range, tackling significant issues like growth, development, transportation, and the needs of older adults. This role involves performing client interviews, intake, and assessments to determine service needs, acting as an agency representative with other service providers, making resource referrals, and providing client advocacy. The position also involves evaluating program eligibility, determining case safety and crisis intervention needs, developing and monitoring client service plans, and coordinating service information exchange with other agencies. The Case Manager will monitor client status, provide counseling and intervention support, conduct on-site visits, perform needs and risk assessments, and escalate cases to supervision as required. Additionally, the role includes preparing and maintaining case documentation, collecting and analyzing data for reports, and providing recommendations. Training and community education, researching community service needs, collaborating with stakeholders, and performing outreach are also key responsibilities. Some positions may be responsible for securing, monitoring, and reporting special program funding. The role also involves providing face-to-face information, consultation, and referral to clients regarding resources for older persons and their families, working with consumers to identify service gaps, and advocating for identified needs. This includes conducting individual needs assessments, working with older adults and their families to identify options, completing care plans, contacting agencies, and following up with clients. Enhancing and maintaining agency resource information, conducting visits with community resource organizations, and providing interpretation/translation services are also part of the role.

Requirements

  • Bilingual English/Spanish required.
  • Bachelor’s degree in gerontology, social work or related human services field and one to three years of related human services experience.
  • OR An equivalent combination of education and experience sufficient to successfully perform the essential duties of the job such as those listed above, unless otherwise subject to any other requirements set forth in law or regulation.

Responsibilities

  • Performs client interviews, intake and assessments to determine service needs and resource requirements.

  • Providing consultation and case coordination for older adults both in person (during a “home visit”) and over the phone.
