About The Position

The Case Manager, ACT Team position involves providing comprehensive case management services to consumers. This includes assessing needs, developing service plans, linking consumers to community resources, and monitoring their progress. The role requires collaboration with treatment teams and outside sources, as well as providing family support and basic living skills instruction. The position also involves maintaining clinical records, managing caseloads efficiently, and responding to crisis situations. A key aspect of the role is to treat consumers with care, dignity, and compassion, respecting their privacy and confidentiality.

Requirements

  • Bachelor’s degree in a mental health discipline.
  • Must complete certification of case management training by Alabama State Department of Mental Health and Mental Retardation post-hire.
  • Must have a valid state-issued driver’s license.
  • Must have proof of current liability insurance.
  • Knowledge in adult specialty to be attained within six months of employment, including:
  • Knowledge of psychiatric disorders in the adult population.
  • Knowledge of cultural diversity.
  • Knowledge of behavior management, crisis intervention and interventions common to acute psychotic consumers, and/or consumers with substance abuse issues as well as, to non-violent crisis intervention practice.
  • Knowledge of Psychiatric Rehabilitation principles.
  • Knowledge of available community resources.
  • Knowledge of educational methods appropriate to adults with severe and persistent psychiatric disorders.
  • Knowledge of rules and regulations regarding appropriate benefit entitlements such as social security, Medicaid, and Medicare.

Responsibilities

  • Assess the consumers’ needs.
  • Identify goals and plans for appropriate services.
  • Provide appropriate linkage to community resources.
  • Monitor consumers to secure placement in the least restrictive environment.
  • Provide ongoing monitoring of consumers’ progress and needs.
  • Advocate on behalf of the consumer to secure community resources.
  • Collaborate with treatment team members and outside sources in the delivery of case management services.
  • Evaluate the consumer for discharge planning.
  • Provide family support and education as needed.
  • Provide basic living skills instruction as needed.
  • Demonstrate the application of adult specific competencies.
  • Seek clinical supervision and consultation as needed.
  • Regularly staff concerns regarding consumers with supervisor.
  • Accept and employ suggestions for improvement.
  • Actively work to enhance case management skills.
  • Document in a timely manner per AltaPointe standards.
  • Document appropriate, complete, and comprehensive assessments.
  • Document measurable and specific goals.
  • Document in a clear and concise manner, addressing goals, interventions used, response to interventions, progress toward goals, and plan for follow through.
  • Document twice weekly staffing notes for consumers on caseload, per ACT standards.
  • Document case disposition, case closings, follow-up and referrals.
  • Document legibly.
  • Document using Avatar appropriately.
  • Obtain signatures as required by supervisor for consumer contact.
  • Maintain mileage reimbursement logs efficiently and with integrity.
  • Effectively manage caseload based on consumer needs.
  • Respond in a timely manner to crisis situations.
  • Make face-to-face contact with consumers on caseload at least once weekly.
  • Meet AltaPointe productivity standards.
  • Treat consumers with care, dignity and compassion.
  • Respect consumer’s privacy and confidentiality.
  • Be pleasant and cooperative with others.
  • Assist consumers and visitors as needed.
  • Ensure personal values do not inhibit ability to relate and care for others.
  • Be sensitive to the consumer’s needs, expectations and individual differences.
  • Be gentle and calm to consumers and families.
  • Actively participate in Performance Improvement activities.
  • Actively participate in AltaPointe committees as required.
  • Complete assigned tasks in a timely manner.
  • Comply with AltaPointe policies and procedures.
  • Attend required in-service training and other workshops.
