About The Position

Individual provides social services assistance/case management to patients and their families. Works collaboratively with the treatment team in aspects of patient care; links patients with needed aftercare resources/referrals; discharge planning; completes, assists with, and/or schedules needed assessments; makes collateral contacts; examines case situations and presents information to medical staff and other members of the treatment team regarding patient’s family and social background pertinent to diagnosis and treatment. Reports to the Program Manager.

Requirements

  • Bachelors of Science or Arts, preferably in social work.
  • Experience working with patients experiencing psychiatric and/or behavioral problems preferred.
  • Knowledge of and experience with psychotropic medication preferred.
  • Eligibility for Social Work License preferred
  • Ability to read and interpret instructions, documents and follow procedures.
  • Ability to write routine reports and speak effectively with patients and their families, employees and public.
  • Ability to solve practical problems and deal with a variety of variables as needed.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Ability to work flexible hours.
  • Ability to work with children, adolescents or adults experiencing mental health disturbances or dual diagnoses.

Responsibilities

  • Serves as a member of the treatment team, maintains team meeting minutes and data (Children’s Unit), and assists with treatment plan review scheduling and notification (Forensics).
  • Coordinates aspects of patient’s treatment and assists with paperwork, referrals, and appointments for patients as needed.
  • Serves as a liaison/contact between patient and family, school, court system, WVDHHR, and other community agencies as needed.
  • Participates/facilitates educational and life skills groups.
  • Coordinates discharge planning, aftercare services, and assists with utilization review.
  • Completes documentation daily.
  • Continues self-knowledge development by participating in staff development (internal and external), ongoing continuing education, and by participation in unit staffings, treatment planning, and training.
  • May assist with admissions as needed.
  • Assists in the orientation of new personnel when requested.
  • Reviews and assures knowledge of new policies and procedures is current.
  • Other duties as assigned.
