About The Position

The Case Manager is responsible for interdisciplinary coordination of care for a designated patient population. The role reviews inpatient admission records to ensure appropriate use of hospital resources and determine whether admission is at the appropriate level of care. The Case Manager identifies discharge needs and coordinates an appropriate discharge plan, while collaborating with the interdisciplinary team to support desired financial and quality outcomes. The role also maintains and enhances relationships with payors.

Requirements

Current BLS certification is required.
Three to five years of inpatient experience.

Nice To Haves

Licensure in the state where the hospital is located is preferred.
Inpatient experience in an acute, IRF, or LTACH setting is preferred.
Professional discipline may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
