About The Position

As a Case Manager, you'll play a vital role in supporting individuals on their recovery journey by connecting them to resources, advocating for their needs, and helping them achieve the goals outlined in their treatment and recovery plans. Positive Resolutions, a program of Resources for Human Development (RHD), provides community-based behavioral health services that help individuals achieve recovery, stability, and independence. Through person-centered support, resource coordination, and community engagement, our team empowers individuals to overcome challenges, access services, and work toward meaningful life goals.

Requirements

Two (2) years of experience providing support to individuals in behavioral health, addiction recovery, unhoused services, or IDD/BHH programs
Intermediate computer skills
Strong communication and organizational abilities
Ability to work independently and in community-based settings

Nice To Haves

Bachelor's Degree in Human Services or a related field
One (1) year of case management experience
Knowledge of behavioral health resources and community support systems

Responsibilities

Work as part of a multidisciplinary team to provide holistic, person-centered services.
Assist individuals in developing and achieving recovery goals.
Coordinate services and connect participants with community resources and supports.
Build strong relationships with community agencies and service providers.
Advocate for participants to ensure their needs are met and barriers are addressed.
Provide community-based case management services throughout the service area.
Transport participants to appointments and community resources when needed.
Complete assessments, service plans, reviews, and required documentation.
Maintain accurate and timely records in accordance with agency and funder requirements.
Collaborate with clinical staff, medical providers, and community partners to support positive outcomes.
