Case Manager, Assertive Treatment Team (ACT)

About The Position

Greater Nashua Mental Health is seeking a compassionate and motivated Case Manager to join its Assertive Community Treatment (ACT) team. The role works directly with individuals experiencing severe and persistent mental illness, helping them access services, build independence, and achieve recovery goals in their communities. The Case Manager provides case management and direct support, coordinates treatment and services with a multidisciplinary team, and supports clients with medication monitoring, crisis intervention, and daily living skills. Services are delivered in community and natural environments to support recovery and independence. The position also includes daily medication runs on a rotating schedule and participation in a rotating on-call schedule once per week.

Requirements

Bachelor’s degree in Social Work, Psychology, Human Services, or a related field.
Strong assessment, diagnostic, crisis intervention, and de-escalation skills.
Strong communication, collaboration, and problem-solving skills.
Ability to work in a fast-paced, team-based environment.

Nice To Haves

1–2 years of related experience preferred.

Responsibilities
