About The Position

Greater Nashua Mental Health is seeking a compassionate and motivated Case Manager to join its Assertive Community Treatment (ACT) team. The Case Manager works directly with individuals experiencing severe and persistent mental illness, helping them access services, build independence, and achieve recovery goals in their communities. Services are delivered in community and natural environments and include case management, direct support, treatment coordination, medication monitoring, crisis intervention, daily living skills support, advocacy, and connections to community resources. The role also involves assisting with daily medication runs on a rotating schedule and participating in an on-call rotation once per week.

Requirements

Bachelor’s degree in Social Work, Psychology, Human Services, or a related field.
Strong assessment, diagnostic, crisis intervention, and de-escalation skills.
Strong communication, collaboration, and problem-solving skills.
Ability to work in a fast-paced, team-based environment.

Nice To Haves

1–2 years of related experience preferred.

Responsibilities

Provide case management and direct support services to individuals with complex mental health needs.
Work within a multidisciplinary ACT team to coordinate treatment plans and services.
Support clients with medication monitoring, crisis intervention, and daily living skills.
Advocate for clients and connect them with community resources.
Deliver services in community and natural environments to support recovery and independence.
Assist with daily medication runs on a rotating schedule.
Participate in a rotating on-call schedule once per week.
