About The Position

Ujamaa Place is seeking a passionate and dedicated Case Manager to initiate its Vocational Rehabilitation Services program in Saint Paul, Minnesota. The role works directly with individuals to help them overcome barriers, build skills, and achieve meaningful employment and long-term self-sufficiency. The position supports Ujamaa Place’s work empowering African American men through services that promote economic independence, stability, and community connection.

Requirements

Bachelor’s degree in Social Work, Human Services, Rehabilitation Counseling, or a related field, or equivalent experience.
Knowledge of community resources and social services available in the Saint Paul/Twin Cities area.
Strong interpersonal and communication skills, with the ability to build trust with diverse populations.
Proficiency in maintaining case documentation and meeting compliance standards.
Commitment to culturally responsive, trauma-informed service delivery.

Nice To Haves

Experience in case management, vocational rehabilitation, or workforce development is preferred.

Responsibilities

Develop and implement individualized vocational rehabilitation plans tailored to each client’s goals and needs.
Conduct comprehensive assessments to identify barriers related to education, housing, mental health, and employment, and determine appropriate support services.
Connect clients with community resources, job training programs, and employment opportunities.
Monitor client progress, maintain accurate case notes, and update service plans as needed.
Collaborate with employers, community partners, and service providers to support client job placement.
Facilitate workshops and one-on-one coaching sessions focused on job readiness, life skills, and career development.
Ensure compliance with program requirements, documentation standards, and reporting deadlines.
