About The Position

Banner Rehabilitation Hospital, in partnership with Select Medical, is hiring a full-time Case Manager Float to cover three inpatient rehabilitation hospital locations in Phoenix, Mesa, and Peoria, Arizona. The position is onsite and scheduled Monday through Friday. The stated starting salary range is $80,000, with increases based on years of experience.

The Case Manager coordinates healthcare decisions using a systematic approach to support treatment plans that improve quality and outcomes, coordinate care across the continuum, promote cost-effective care within the allotted timeframe, ensure hospital-based services meet patient-related utilization management criteria, and implement safe and appropriate discharge plans. The role also assesses patients’ psychosocial needs and provides intervention as part of discharge planning.

The hospitals provide inpatient rehabilitation for people facing medical, physical, emotional, and vocational challenges related to brain injuries, spinal cord injuries, neurological disorders, orthopedic issues, amputation, and multiple traumas. Requirements

• Current licensure per state guidelines in a clinical or related discipline, or a bachelor's or master's degree in a health or human services discipline.
• Previous experience in hospital or skilled nursing facility case management and discharge planning is stated in the overview.

Nice To Haves

• Previous experience in Case Management and Discharge Planning.
• CCM certification.

Responsibilities

• Coordinate healthcare decisions to support treatment plans that improve quality and outcomes.

• Cover the Banner Rehabilitation Hospital locations in Phoenix, Mesa, and Peoria, Arizona.
