This is an outpatient community-based program serving clients (ages 18 plus) with services including Housing, Education, and Vocational Support, Individual and family Counseling, and Medication management and support services. Requirements

  • Associate degree in a social service field plus two (2) years of experience working in a mental/behavioral health field required
  • Valid California Driver’s License required and eligibility to meet agency driving criteria required

Nice To Haves

  • Internship may be substituted for a portion of the work experience
  • Bachelor’s degree in a social science field plus six (6) months of experience preferred.

Responsibilities

  • Provides services at the office, in the community, and in clients’ homes.
  • Uses a personal vehicle to drive within the community to provide services and to transport clients in the personal vehicle.
  • Provides case management services to clients and their families, as deemed necessary and appropriate through the assessment process and in collaboration with the treatment team.
  • Participates in meetings, such as Treatment Team Meetings, to ensure clients’ needs are met and recommendations are followed through.
  • Documents all services provided clearly and concisely within the Electronic Medical Record within the timelines and framework established by Stars.
  • Assists the primary therapist with initial and ongoing assessments of the client and family to identify strengths and needs.
  • Monitors the clients’ progress toward goals outlined in their treatment plans.
  • Informs the primary therapist of progress.
