About The Position

As a Case Manager, you will provide information, referrals, advocacy, and case assistance services to older adults participating in JASA’s Older Adult Centers in Queens. Through in-person interactions, you will help connect participants to critical resources and services that promote independence, stability, and overall well-being. This role involves working at both the Roy Reuther Older Adult Center and the Rockaway Park Older Adult Center, with a 35-hour work week spread across Monday to Friday.

Requirements

  • Graduate of an accredited college or university with a B.S.W., B.A., or B.S. degree, or equivalent work experience.
  • Strong oral, written, and record-keeping skills.
  • Basic computer literacy and ability to learn and use program-specific software.
  • Ability to communicate effectively in spoken and written English.
  • Understanding and appreciation of diversity, including race, religion, ethnicity, sexual orientation, and individual values in serving clients and families.
  • Availability to work evenings and/or weekends as required for emergency operations or City-funded initiatives.

Nice To Haves

  • Prior group leadership experience preferred.
  • Knowledge of Spanish, Russian, or Chinese dialects is a plus.

Responsibilities

  • Provide information regarding health, welfare, educational, social, and recreational services available to older adults.
  • Screen clients for eligibility for benefits and entitlements and assist with applications and advocacy efforts.
  • Assist clients with accessing services such as transportation, home care, home-delivered meals, Medicare, Medicaid, emergency assistance, legal aid, housing resources, and healthcare services.
  • Explain and interpret agency programs and available resources to participants.
  • Inform supervisors of crisis situations or circumstances requiring additional guidance.
  • Conduct community education workshops and support groups.
  • Work closely with community organizations and service providers to meet client needs.
  • Participate in training sessions and unit meetings.
  • Complete required case records, reports, and statistics using secure electronic databases.
  • Maintain accurate and timely documentation of services provided.
  • Perform additional related duties and assignments as requested.
  • Participate in Cooling Center Hours, Make-Up Days, or other emergency operations as required by City funders.
  • Be available to work extended hours or weekends when necessary to support older adult center members.
