About The Position

Legacy Behavioral Health is hiring individuals with a passion for making a difference in their community! We are seeking a Case Manager to work in Valdosta, Georgia. The Case Manager, under general supervision performs a wide range of tasks in the community to assist consumers in regaining control over their own lives while promoting the recovery process. Utilizing psychiatric rehabilitation interventions provides skills training, education and case management to consumers and their families. Monitors consumer progress towards treatment goals. Extensive documentation is required; accurately charts services and consumer progress. Must be able to work well with community agencies and function as part of a team.

Requirements

• Bachelor’s degree in social work, psychology, mental health, nursing, education, sociology, criminal justice, counseling, rehabilitation counseling, guidance counseling or an equivalent field.
• One year of full-time experience in social services delivery.
• Knowledge of mental illness.
• Valid Georgia driver’s license.

Responsibilities

• Perform a wide range of tasks in the community to assist consumers in regaining control over their own lives while promoting the recovery process.
• Utilize psychiatric rehabilitation interventions to provide skills training, education, and case management to consumers and their families.
• Monitor consumer progress towards treatment goals.
• Accurately chart services and consumer progress.
• Work well with community agencies and function as part of a team.
• Provide monitoring, educational and/or crisis handling.
• Travel to off-campus locations in LBHS or personal vehicle.
