About The Position

The Case Manager is responsible for coordinating the development and implementation of each person’s programs, various support services, and training for people with intellectual disabilities. This role involves ensuring compliance with guidelines, maintaining client eligibility, and fostering therapeutic relationships by encouraging family involvement, monitoring living and work settings, and ensuring community integration. The Case Manager also provides direct care as needed and maintains good relationships with staff to ensure programming meets standards. This position may also involve covering for other Case Managers and performing other assigned duties.

Requirements

  • High school diploma/equivalent required
  • Must be able to work in an office (not a remote role) and travel for company business as needed
  • Valid Texas driver’s license and related insurance
  • Knowledge of inventory and supply ordering, distribution, and control processes and practices
  • Proficient computer skills and ability to manage inventory, supply ordering, and accounting/cost software
  • Proficient ability to maintain strict confidentiality
  • Strong communication, interpersonal, organizational, and time management skills
  • Strong attention to detail, ability to maintain precise standards, and follow established processes
  • Strong critical thinking and analytical skills
  • Ability to work independently or as part of a team

Nice To Haves

  • 1-2 years of experience in an equivalent role preferred

Responsibilities

  • Coordinate program plans by leading meetings and documenting goals and ensure annual review of plans.
  • Deliver program plans by placing training documents in service locations and training staff on implementation and data collection.
  • Monitor goals by overseeing staff interactions and data collection.
  • Integrate services by finding community services that accept Medicaid.
  • Record progress by directing staff in data collection and reviewing, and adjusting goals based on progress.
  • Communicate with families and agencies.
  • Develop pre-discharge and pre-admission plans.
  • Ensure compliance with guidelines.
  • Submit paperwork to the billing department.
  • Maintain client eligibility.
  • Maintain therapeutic relationships by encouraging family involvement, monitoring living and work settings, and ensuring community integration.
  • Provide direct care as needed.
  • Maintain good relationships with staff.
  • Ensure programming meets standards.
  • Cover for other Case Managers.
  • Other duties assigned.
