About The Position

Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. The role is based on the Assertive Community Treatment (ACT) model, STRIDES of Alameda County is an intensive community support service for 120 individuals with severe mental illness (SMI), between the ages of 18-62, who are considered to be at great risk for psychiatric hospitalization.

Requirements

High School Graduation or G.E.D. equivalent and three (3) years of direct services in mental health or community services; OR Associate’s degree and two (2) years of experience; OR Bachelor’s degree in Social Services
At least 1 year of direct service experience providing assistance as a Vocational Specialist
Valid and current driver’s license
Personal vehicle insurance with your name listed as a driver
Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
