About The Position
Memorial Medical Center is hiring a Case Manager Social Worker for an as-needed PRN role in its behavioral health unit. The position supports patients and their families by assessing needs, arranging services, coordinating with the healthcare team, and assisting with discharge planning. The role also includes mentoring new staff, students, and interns, performing Medicaid screenings for NHP when indicated, and helping patients with Safety Net applications.
Requirements
- Bachelor’s degree in social work.
- Licensed Social Worker within six months of hire or transfer date.
- Basic Life Support (BLS) certification within 30 days of hire.
Responsibilities
- Interview and assess patients and/or their families, caregivers, and legal representatives.
- Determine, prioritize, provide, and/or arrange needed internal and external services and interventions.
- Participate in case reviews to evaluate case management and patient progress.
- Consult with healthcare team members to promote, monitor, and evaluate compliance with patients’ treatment plans.
- Assist with discharge planning and processes.
- Identify appropriate discharge resources, including transportation, housing, healthcare, and social or spiritual services, and provide referrals.
- Mentor new staff, students, and interns.
- Perform Medicaid screenings as indicated for NHP.
- Assist patients with Safety Net applications when needed.
