About The Position

YouthLink is a recognized leader in the Twin Cities and beyond for our work to build pathways of opportunity for homeless youth and youth in crisis ages 16 – 24. Our model is based on providing support through outreach, handling basic needs, intervention, and aftercare. Our youth-focused team provides innovative, evidence-based services that guide youth on a holistic level toward futures of education, employment, health and wellness, housing stability and aftercare. We are looking for a Case Manager – Scattered Site Housing (RRH) to join our team and help us advance our mission. The Case Manager – Scattered Site Housing provides support, guidance, and coordination of wrap around services for youth ages 18-24 who have been systems-involved (corrections, foster care) with the goal of stabilizing them in housing. Working in collaboration with Hennepin County In Reach staff, the Director of Housing and the Finance team, the Case Manager-Scattered Site Housing assists youth with obtaining self-sufficiency through a holistic approach, including but not limited to appropriate referrals for housing, counseling, financial counseling, job readiness programs, and higher education. Lastly, this position will also be expected to engage with our community partners to increase additional services our clients may need. This position is meant to be flexible and work with any client coming into YouthLink. At this time YouthLink does not offer relocation or US work visa sponsorship.

Requirements

  • 1+ years’ case management experience.
  • 2 years’ experience working with at-risk young adults.
  • 2 years’ experience working with homeless youth.
  • 2 years’ experience with community engagement.
  • Valid MN State Driver’s License, insurance.
  • Comfort with Microsoft Office applications as well as exhibit an ability to learn and utilize handheld applications systems and other electronic recording systems.
  • Must be able to come into work on time, for all regularly scheduled work shifts.
  • Able to manage time effectively and work a flexible schedule.
  • Ability to work efficiently and effectively both individually and as part of a professional team.

Responsibilities

  • Provides support, guidance, and coordination of wrap around services for youth ages 18-24 who have been systems-involved (corrections, foster care) with the goal of stabilizing them in housing.
  • Assists youth with obtaining self-sufficiency through a holistic approach, including but not limited to appropriate referrals for housing, counseling, financial counseling, job readiness programs, and higher education.
  • Engage with our community partners to increase additional services our clients may need.
  • Provide non-judgmental case management based on a harm-reduction/trauma-informed approach.
  • Work with a young adult on caseload to decrease life barriers.
  • Able and willing to learn and incorporate agency philosophy into service delivery.
