About The Position

Housing Works is seeking a Housing Case Manager for its Emergency & Transitional Housing Program (ETH). This program provides time-limited transitional housing and support services to individuals reentering the community after incarceration or other justice system involvement. Participants receive onsite services including case management, vocational training, housing advocacy, and mental health support to address barriers to community housing and build skills for stable functioning. The Housing Case Manager will provide direct case management and housing placement assistance, including psychosocial assessments, co-developing treatment and housing plans, connecting individuals to community-based services, assisting with benefits enrollment, and helping participants find and prepare for permanent housing. This role is ideal for professionals with case management or housing placement experience focused on improving outcomes for justice-involved individuals.

Requirements

• Bachelor’s Degree required in social services, psychology, social work or other related field.
• Experience providing case management or direct client service.
• Experience working with individuals involved in the criminal legal system, as well as individuals with substance use or mental health diagnoses.
• Strong written documentation and communication skills
• Good problem-solving skills and ability to work autonomously
• Microsoft Office Suite, particularly Excel, required
• Experience utilizing EMR systems.

Responsibilities

• Manage a caseload of approximately 35-45 clients enrolled in the ETH Program.
• Conduct psychosocial assessments with all newly enrolled clients.
• Conduct housing eligibility screenings with clients to inform and develop the housing plans of care.
• Co-develop robust Treatment Plans and Housing Plans with clients assigned to caseload and review every 30 days.
• Meet with clients regularly in accordance to program workflow and contract deliverables.
• Assist clients with obtaining, or retrieving, vital documents such as a government issued IDs, social security cards, birth certificates, etc.
• Assist clients with benefits and entitlements enrollment.
• Assist clients with housing applications and scheduling apartment viewings.
• Work with clients to assist with family reunification.
• Connect clients to internal and external health, wellness or other support services.
• Escort clients to service appointments as needed and assist with coordinating alternative transportation services.
• Engage in care coordination activities with internal and external providers.
• Conduct case conference meetings with clients interdisciplinary care teams.
• Engage in crisis intervention as needed.
• Maintain accurate and up-to-date client charts utilizing Housing Works electronic medical records systems.
• Adhere to program and agency quality assurance standards.
• Participate in quality improvement activities.
• Submit daily, weekly, monthly and ad hoc reports in a timely manner.
• Participate program and agency meetings.
• Promotes Housing Works’ core values in day to day activities.
• Performs other tasks as assigned by supervisor.
• Provide culturally competent services in accordance to Housing Works policies and practices.
• Engages in advocacy efforts as outlined in Housing Works’ and the Justice Initiatives Department policies.
• Ability to work with clients with diverse clients with varying levels of service needs.
• Works collaboratively with other team members within the Justice Initiatives Department and cross-agency.
• Attend and participate in staff meetings and trainings as scheduled.
• Responsiveness to emails, texts and phone calls.
• Flexibility to work pre-scheduled evening hours as needed to meet client service deliverables.
