About The Position

This position is responsible for completing departmental orientation, initial and annual competencies. The Case Manager will collaborate with interdisciplinary staff to identify and resolve barriers to care or discharge. They will complete documentation in accordance with workflow timelines and content standards, including the Individual Plan of Care (IPoC) per CMS guidelines. The role involves scheduling and conducting family conferences and caregiver communications following team meetings or as needed. Additionally, the Case Manager will facilitate weekly care team conferences to review patient progress and adjust treatment plans, review the assigned Case Mix Group (CMG) and assist the care team in identifying and documenting treated comorbid conditions, and notify the Health Information Management (HIM) team as needed. Other duties may be assigned.

Requirements

  • Bachelor’s or Master’s degree in Social Work, or as required by state regulations.
  • Minimum of 2 years of social work or case management experience in an inpatient setting highly preferred.
  • Experience in acute or rehabilitation hospital preferred.

Nice To Haves

  • Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).
