About The Position

Lancaster Rehabilitation Hospital is a state-of-the-art, 59-bed inpatient acute rehabilitation hospital dedicated to the treatment and recovery of individuals who have experienced the debilitating effects of a severe injury or illness. Our rehabilitation programs provide ongoing care and specialized treatment to patients throughout their recovery journey. Requirements

  • Bachelor’s or Master’s degree in Social Work, or as required by state regulations (verify state-specific requirements).
  • Minimum of 2 years of social work or case management experience in an inpatient setting highly preferred; experience in acute or rehabilitation hospital preferred.

Nice To Haves

  • Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).

Responsibilities

  • Complete departmental orientation as well as initial and annual competencies.
  • Collaborate with interdisciplinary staff to identify and resolve barriers to care or discharge.
  • Complete documentation in accordance with workflow timelines and content standards, including the Individual Plan of Care (IPoC) per CMS guidelines.
  • Schedule and conduct family conferences and caregiver communications following team meetings or as needed.
  • Facilitate weekly care team conferences to review patient progress and adjust treatment plans.
  • Review the assigned Case Mix Group (CMG) and assist the care team in identifying and documenting treated comorbid conditions; notify the Health Information Management (HIM) team as needed.
  • Perform other duties as assigned.
