Case Manager Registered Nurse – Louisiana Field
About The Position
Coordinates case management activities with members to evaluate medical needs and facilitate their overall wellness. Develops strategies to address issues and improve outcomes, and identifies opportunities to enhance members’ overall wellness through integration. Instructs members on programs and procedures in compliance with network management and clinical coverage policies. The position is based in the field and requires residence in one of the specified Louisiana parishes, travel within 125 miles round trip for one day in the field, and a variable work schedule that includes working until 9 p.m. local time two days per week.
Requirements
- Current, active registered nurse clinical licensure.
- Case management experience.
- Strong customer service orientation and problem-solving skills.
- Excellent communication and telephonic skills.
- Excellent motivational interviewing skills and ability to build rapport and trust telephonically.
- Excellent documentation skills and ability to respect compliance and regulatory standards.
- Highly organized and self-driven without direct supervision.
- Effective time-management skills.
- Ability to achieve performance metrics.
- Technology proficiency.
- Must reside in one of the following Louisiana Region 4 parishes: Evangeline, St. Landry, Acadia, Lafayette, St. Martin, Iberia, or Vermilion.
- Ability to travel within 125 miles round trip for one day in the field.
- Ability to work a variable schedule, including two days weekly until 9 p.m. local time.
Nice To Haves
- Bachelor’s and/or master’s degree.
- Certified Case Manager (CCM) certification.
- Previous experience in a managed care organization.
