About The Position

The MSW Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive, complex, and safe discharge plans. Collaborates with RN Case Manager's for guidance with medically complex cases. The MSW Case Manager is also responsible for facilitating coordination of care. The MSW Case Manager along with the other members of the healthcare team accepts responsibility, authority, and accountability for management of care. This shall be in accordance with the current policies and procedures, mission, vision, and values of UVA Health Northern VA Culpeper. Serves as a resource for discharge planning and care coordination for psychosocially complex cases. Maintains current knowledge of available community resources and post-acute care options.

Requirements

  • Master's Degree in Social Work from school accredited by the Council on Social Work Education.
  • 2 or more years of professional social work experience or 2 or more years of healthcare experience preferred.
  • Case Management or related experience preferred.
  • Weekend and holiday coverage.
  • Proficient communicative, auditory, and visual skills.
  • Attention to detail and ability to write legibly.
  • Ability to lift/push/pull 100 lbs. For any weight over 35 lbs., use Safe Handling Equipment.

  • Knowledge/Competence of Microsoft Office products and keyboarding.

Responsibilities

  • Serve as a leader of the complex discharge planning process.
  • Collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive, complex, and safe discharge plans.
  • Collaborate with RN Case Managers for guidance with medically complex cases.
  • Facilitate coordination of care.
  • Accept responsibility, authority, and accountability for management of care in accordance with current policies and procedures, mission, vision, and values of UVA Health Northern VA Culpeper.
  • Serve as a resource for discharge planning and care coordination for psychosocially complex cases.
  • Maintain current knowledge of available community resources and post-acute care options.
  • Communicate effectively with people of diverse backgrounds and all levels of authority.
  • Provide communication and updates to patients and guests and ensures their needs are being met, when applicable.
  • Respond quickly and properly to irregular and emergency situations.
  • Review processes, identifying and recommending opportunities for improvement.
  • Monitor the quality of core Public Safety processes.
  • Hold subordinates accountable for customer service standards and initiates remediation when needed.
  • Maintain confidentiality in all matters related to patient care and visitor issues.
  • Actively participate in identifying areas of opportunity to improve the experience for patients and guests.
  • Provide cultural information to facilitate support for a treatment plan.
  • Ensure that information pertaining to the patient's outpatient services, hospitalization, (i.e., advance directives, consents from treatment, diagnosis and prognosis, treatment plan, follow-up appointments and Health Care Education) are accurately communicated.
  • Perform additional duties as assigned.
