About The Position

Sentara Martha Jefferson is hiring an Inpatient Case Manager- MSW for a Full Time, Day shift position in Charlottesville, VA. The Integrated Case Management team at Sentara Martha Jefferson has 16 members including MSW and RN Case management staff. This is a collaborative environment with a patient- first mind set. The average cases per day are 15, and a strong sense of urgency is preferrable. The hours are primarily between 8am and 4:30pm with some variation and one weekend shift per 6 to 8 weeks. The Inpatient Case Manager-MSW is responsible for the provision of medical social work services to patients receiving care in the hospital setting. Assesses the social, cultural, environmental, and financial situations, as well as the disposition needs of each referred patient. Collaborates with healthcare team members for the patient and community organizations to identify needs and resources in the hospital and community. In consultation with the healthcare team develops effective transition plans. Collaborates with the RN Case Manager to ensure a smooth flow of patients through their hospital stay. Functions in one of the following practice settings: Hospitals Only.

Requirements

  • Master degree in Social Work
  • Certification in Case Management (CCM, CMAC, or ACM-SW) is required within one year of eligibility.
  • BLS is required within 90 days of hire.
  • 1 year of acute care and/or healthcare experience with either educational internship experience in an acute care facility required.
  • Collaboration in a team environment required.

Nice To Haves

  • Experience with community resources is preferred.
  • Develops effective transition plans preferred.
