About The Position

The MSW Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive, complex, and safe discharge plans. Collaborates with RN Case Manager's for guidance with medically complex cases. The MSW Case Manager is also responsible for facilitating coordination of care. The MSW Case Manager along with the other members of the healthcare team accepts responsibility, authority, and accountability for management of care. This shall be in accordance with the current policies and procedures, mission, vision, and values of UVA Health Northern VA Culpeper. Serves as a resource for discharge planning and care coordination for psychosocially complex cases. Maintains current knowledge of available community resources and post-acute care options.

Requirements

  • Master's Degree in Social Work from school accredited by the Council on Social Work Education.
  • 2 or more years of professional social work experience or 2 or more years of healthcare experience preferred.
  • Case Management or related experience preferred.
  • Weekend and holiday coverage.
  • Proficient communicative, auditory, and visual skills.
  • Attention to detail and ability to write legibly.
  • Ability to lift/push/pull 100 lbs. For any weight over 35 lbs., use Safe Handling Equipment.

  • Knowledge/Competence of Microsoft Office products and keyboarding.

Responsibilities

  • May be asked to perform additional duties as assigned.
  • May require the use of safety equipment, such as HEPA mask, for infection prevention.
  • On call responsibilities as directed.
  • Ability to travel between campus buildings, remote facilities, and out of town as needed.

Benefits

  • The base compensation range for this role is $28.68 - $46.57 hourly.
