About The Position

Canvas Health is a non-profit, Certified Community Behavioral Health Clinic (CCBHC) specializing in serving clients who are on Medicaid. The mission of Canvas Health is to bring hope, healing, and recovery to people's lives. Our integrated programs and services offer coordinated care to people of all ages in their homes, the community, and in our clinics. We strive for excellence in clinical care as we serve those living with mental illness, substance use disorders, crisis, unstable housing, and trauma. The role of the MCHN Case Manager is to work in partnership with the other members of the multi-discipline treatment team to address the physical health, mental health and overall well-being needs of assigned clients. This work shall be done in a person-centered, trauma-informed manner that may equal sharing responsibilities across positions at times when client needs arise. The primary responsibility of the Case Manager will be to ensure coordination of care occurs, referrals are completed, and services are monitored over time and to provide therapeutic skills interventions to adults to assist them in engaging in community based mental health, chemical health and physical health care.

Requirements

  • Bachelor's degree in social work or in the human services field.
  • Two years' experience preferred working with mentally ill and/or chemically dependent individuals, of which at least one year is experience in community-based mental health, chemical health treatment with adults recovering from mental illness/chemical dependency or combination of equivalent experience and training.
  • Experience working with and/or delivering services to diverse populations.
  • Knowledge of local social and human service networks.
  • Experience with Electronic Health Record systems.

Nice To Haves

  • Bachelor's degree or master's degree in social work or other human services field.
  • At least one year in a case management role or similar role
  • Experience working independently with clients with various mental health/ chemical health concerns
  • Experience working in crisis settings or with clients in crisis situations
  • Experience with social service providers and referrals in one or more counties in the metro area
