About The Position

ServiceSource is seeking a Case Manager (Intake Coordination) to serve a transformative program supporting Fairfax County families on their path toward housing stability and long-term self-sufficiency. In this role, you will conduct participant intakes and assessments, identify immediate needs and barriers to success, connect individuals and families with critical community resources, coordinate referrals and service navigation, and develop participant-centered plans that support employment and self-sufficiency goals. You'll combine the flexibility of a hybrid work environment with the rewarding experience of engaging directly with participants and community partners throughout Fairfax County. Case Managers play a critical role in the multi-year contracted program designed to help Fairfax County Redevelopment and Housing Authority (FCRHA) households stay stably housed while building the income, skills, supports, and confidence needed to move toward long-term self-sufficiency. As an integral part of the support team, the Case Manager works to move individuals toward self-sufficiency by linking individuals and families to essential community resources, coordinating referrals and applications, housing stability supports, and assistance in navigating barriers to employment. Using a wrap-around approach, the Case Manager develops care plans, continuously assesses changes in status and support needs, and updates care plans as necessary to ensure ongoing support.

Requirements

• Bachelor’s degree in social work, psychology, or human services preferred.
• Associate degree (AA/AS) in Human Services or a related field with three years of relevant experience; or high school diploma or GED with a minimum of five years of relevant experience required.
• Experience working with vulnerable populations, including households receiving TANF, living in subsidized housing, individuals with disabilities, immigrants, or justice‑involved preferred.
• Demonstrated ability to conduct structured interviews, complete assessments, identify immediate needs, and document findings accurately.
• A minimum of 3-5 years of experience in case management, intake and eligibility coordination, care coordination, and the development and implementation of individualized plans is preferred.
• Annual criminal background checks, child abuse clearances, and other required screenings in compliance with ServiceSource, state, and funding agency regulations.
• Availability and willingness to travel weekly within the local area to perform the job.
• Availability and willingness to work flexible hours, including evenings and/or weekends, when needed.
• Valid driver’s license and/or access to reliable transportation to perform work-related travel required. Eligible drivers must have a good motor vehicle record (MVR).
• Proficiency in Microsoft Office products and the ability to become familiar with ServiceSource specific programs and software.
• Strong verbal and written communication skills.

Responsibilities

• Schedule and Conduct participant intake and enrollment, complete baseline assessments using the Employment Barriers Screening Tool and HUD Self-Sufficiency Matrix, and identify immediate participant and family needs.
• Facilitate smooth transitions to ongoing case management and ensure timely follow-up and continuity of services.
• Complete comprehensive assessments and coordinate services with internal and external partners to support participant success.
• Develop, implement, and regularly update person-centered plans focused on self-sufficiency and employment goals.
• Connect participants to community resources, assist with service navigation, and maintain accurate documentation to support compliance, reporting, and outcomes tracking.
• Build and maintain long-term relationships with community partners, participants, their families, and other stakeholders (e.g., medical teams, paid and natural supports, legal guardians) to ensure access to up-to-date resources and support.
• Regularly communicate and collaborate with FCRHA support staff.
• Stay informed on current and emerging community resources, including programs and services related to healthcare, housing, education, employment, and other relevant supports.
• Build and maintain partnerships with local organizations to ensure access to the most up-to-date information, enabling effective referrals and support for individuals and families.
• Record participants’ progress including charting referrals, home visits and other notable interactions
• Facilitate and participate in team meetings.
• Identify and/or provide support resources related, but not limited to independent living, aging in place, medical support, mental health support, emergency response plans/support, in-home companion care, housing, social and leisure activities, transportation, and financial planning and needs.
• Actively seek opportunities to enhance knowledge of systems supporting FCRHA recipients, including the barriers they face. Foster collaboration through participation in community meetings, professional networks, and relevant trainings, enabling effective referrals and comprehensive care for individuals and families.
• Regularly meet with the Director of Employment Services and key team members, including case managers, youth transition specialists, and employment development specialists from ServiceSource and affiliated organizations, to ensure alignment and progress toward grant objectives
• Perform other responsibilities as assigned.
