ServiceSource is seeking a Case Manager (Intake Coordination) to serve a transformative program supporting Fairfax County families on their path toward housing stability and long-term self-sufficiency. In this role, you will conduct participant intakes and assessments, identify immediate needs and barriers to success, connect individuals and families with critical community resources, coordinate referrals and service navigation, and develop participant-centered plans that support employment and self-sufficiency goals. You'll combine the flexibility of a hybrid work environment with the rewarding experience of engaging directly with participants and community partners throughout Fairfax County. Case Managers play a critical role in the multi-year contracted program designed to help Fairfax County Redevelopment and Housing Authority (FCRHA) households stay stably housed while building the income, skills, supports, and confidence needed to move toward long-term self-sufficiency. As an integral part of the support team, the Case Manager works to move individuals toward self-sufficiency by linking individuals and families to essential community resources, coordinating referrals and applications, housing stability supports, and assistance in navigating barriers to employment. Using a wrap-around approach, the Case Manager develops care plans, continuously assesses changes in status and support needs, and updates care plans as necessary to ensure ongoing support.
