About The Position

The Salvation Army is seeking a Case Manager III for its Owens Campus. This full-time, non-exempt position is crucial to our mission of preaching the gospel of Jesus Christ and meeting human needs without discrimination. The Case Manager III will manage over 30 clients with complex needs, including disabilities, long-term homelessness, or institutional histories. This role involves coordinating with various healthcare providers, leading subsidy and navigation efforts, training junior staff, and conducting outreach in challenging areas. The position requires direct case management services to homeless and at-risk veterans and their families, screening eligibility, facilitating intake, and coordinating care with VA personnel and other community partners. The Case Manager III will also assist veterans with employment and training goals, identify and facilitate mainstream needs, and alleviate employment barriers. Maintaining client records, data entry into the HMIS system, and ensuring participant confidentiality are key responsibilities. The role also involves performing interventions, crisis-oriented counseling, and conducting outreach activities in various facilities and home visits. This position has access to confidential information and is subject to grant funding, meaning the position could be eliminated if funding ceases. Confidentiality and privacy must be observed at all times.

Requirements

  • Bachelor’s degree in social work, psychology, public health, or related field required.
  • Minimum 4 years of progressively responsible case management experience.
  • Demonstrated knowledge of veteran health care systems and housing-first models.
  • Experience mentoring staff and coordinating complex client care.
  • Strong oral and written communication skills.
  • Proficiency in Microsoft Office Software (Word, Excel, etc.).
  • Ability to communicate and work effectively with a diverse group of residents, staff, and community partners.
  • Must have a valid driver's license, a clean driving record, and own a registered and insured vehicle for client visits.
  • Must pass a criminal background check.

  • Relevant experience in senior case management or veteran services may substitute for formal education.

Responsibilities

  • Manage 30+ clients with complex needs, including disabilities, long-term homelessness, or institutional histories
  • Coordinate with hospitals, detox units, and behavioral health providers
  • Lead SSVF’s shallow subsidy and healthcare navigation efforts
  • Train and support junior case management staff
  • Conduct outreach in rural and extreme high-risk areas (e.g., Wetlands Park, Laughlin, Logandale)
  • Provide direct case management services to homeless and at-risk veterans and their families.
  • Screen veterans’ eligibility for the program and facilitate the intake process.
  • Coordinate care with VA personnel and others on behalf of participants for eligibility documentation, mental and physical health services.
  • Learn and maintain knowledge of community employment agencies and procedures, along with modifications to those procedures.
  • Assist veterans in identifying education and training goals and developing plans to meet those goals.
  • Assist veterans in identifying mainstream needs and facilitate the process with appropriate service providers to meet those needs.
  • Assist veterans and their families to alleviate employment barriers.
  • Maintain, record, and report the progression of veterans and their families through program participation.
  • Perform interventions and crisis-oriented counseling.
  • Conduct outreach activities, including visits to facilities such as shelters, treatment programs, soup kitchens and other locations where homeless and at-risk families may congregate.
  • Conduct home visits to provide case management services.
  • Maintain client databases and referrals for services and generate reports.
  • Complete accurate and timely data entry into the HMIS system.
  • Always maintain participant confidentiality.
  • Attend all scheduled team meetings and training sessions.
  • Other duties and special projects as assigned.
  • Serve as lead navigator in drug-impacted zones or encampments with high health risk.
  • Re-engage clients who repeatedly exit housing or services.
  • Collaborate with all community partners on health, housing, and justice-involved veteran reintegration.
